Hope it's OK to reply to this old thread, but we started having 3somes about 4 years ago.  The first was with a girlfriend of mine who I hadn't seen for years, who was staying with us for the weekend. The kids had gone to bed, and we were all drinking home made cocktails. We'd been given the game "Cards against humanity" the previous Christmas but when we got the box out, it needed 4 players.
